Medical report form
A Medical Report is required when your child enters our school. Your family doctor will need only a few minutes to fill out the form which we provide for that purpose. These forms are necessary; therefore, we cannot allow a child to attend our school without a medical report. After the original report is on file, you must update the immunizations when your child receives additional shots.
OTC permission to administer medication form
In order to administer anyover-the-counter oral medication we must have a written doctor’s note stating the dosage, frequency and our medicine form, which gives parental permission. This includes medications that stat “consult a physician if your child is under 2 years of age.”
Prescription medication administration permission form
Prescribed medications must be stored in the original containers in which they were dispensed with the pharmacy labels. Prescription medications shall be administered only to the child for whom they were prescribed, using amount and frequency of dosage specified on the prescription label. Pharmaceutical samples must be stored in the manufacturer’s original packaging, labeled with the child’s name, and must be accompanied by written instructions specifying: the child’s name; the names of the medication; the amount and frequency of dosage; the signature of the prescribing physician or other health professional; and the date the instructions were signed by the physician or other health professional.
